What We Do

Accident Investigations

Our health and safety experts can conduct a comprehensive accident investigation following an incident, injury or fatality at work.

At Pragma, we have accumulated a wide range of industry experience investigating serious and complex accidents.  Our team has investigated accidents in many sectors and in a wide variety of premises.

The HSE’s annual statistics continue to show that accidents at work still present a very real threat to the UK’s workforce.

It is essential you instruct someone who genuinely understands health and safety to undertake your accident investigations.  Only an expert has the experience needed to identify all of the contributory factors as well as the ultimate reasons for the accident.  Without the complete picture, you will not be able to put everything in place to provide the safest working environment for your employees.

Why should you choose us to conduct your next accident investigation?

We have the health and safety expertise to conduct a thorough, balanced and independent assessment of the circumstances surrounding a fatal or serious accident.

  • We have a wide range of industry experience. Our team has investigated accidents in many sectors and in a wide variety of premises.  This experience has been built up during our time working in industry as well as consultants.
  • We have practical experience in carrying out fatal and serious accident investigations. We have also provided support for clients in dealing with HSE Improvement Notices, thereby preventing prosecution.  We have helped in providing pragmatic solutions to minimise the likelihood of reoccurrence.
  • We have also helped clients by supporting them with the process of defending civil claims, which has avoided significant financial settlements needing to be paid.
  • We understand how HSE inspectors think because many of our team were inspectors themselves.
  • Our Managing Director was part the Working Group that produced the IOSH guidance “Learning the lessons: How to respond to deaths and other serious incidents”.
  • Our experience as expert witnesses will also add value as we are familiar with gathering and presenting evidence to support a legal case and understand how best to liaise with the relevant regulatory bodies.

The Management of Health and Safety at Work Regulations 1999 and The Social Security (Claims and Payments) Regulations 1987 explain that you have a responsibility to carry out accident investigations to establish the background behind the accident so that you can take the required measures to minimise the likelihood that similar incidents will happen again.

It is important to remember that, even if the need for the investigation has been brought about by a civil claim or as a result of an Improvement Notice or a Prohibition Notice, the objective is not to apportion blame but to find the root causes of the accident.

The ability to put things right also offers another direct benefit.  If you can minimise the likelihood of reoccurrence you will also minimise the likelihood that you will face more claims or even prosecution in the future.
